We are required to provide certain information to you - listed in the sidebar to the left - before your visit. When you arrive at DEC for your appointment, we will ask you to sign acknowledgement of receipt of our policies. Before your visit to DEC, please take time to review these policies. This should clarify questions you may have and will make the registation process faster.
