Registration

To prepare for your visit to the DALLAS ENDOSCOPY CENTER LLC, your medical history and insurance information is needed.   Please bring your photo id and insurance cards.  All Medicaid patients need to bring their current month's card with them to the Center.

You will be notified prior to your procedure regarding your deductable co-insurance and co-pay responsibility.
